You may be eligible to take part in a research study. This form gives you important information about the study.  It describes the purpose of the research, the risks and possible benefits of participating in the study. 

Purpose of this Study

You are invited to participate in a research study that explores Global Pentecostalism through Pastor Benny Hinn’s career. The purpose of this study is to explore the way Hinn and his particular approach to faith healing has moved around the globe in dynamic ways, riding various networks, developing relationships, and being ever changed by global encounters. This study is being conducted by Dr. Nathanael J. Homewood.

You have been asked to participate in this study because either 1.) You have or will have interacted with Benny Hinn in some way (including but not limited to his television program, books and literature, online presence, crusades, healing services, pastor’s conferences); and/or 2.) You have insight into the global transmission of Pentecostalism.  As such, your participation in this study is a valuable asset that can contribute significantly to a deeper understanding of how Pentecostalism moves throughout the world in dynamic ways.

Procedures / What will happen to me in this study? 

If you decide to participate, you will complete the online form. Completion of this form does not obligate you to participate in any other format. However, you may choose to volunteer to be interviewed about your perspectives and experiences concerning Pentecostalism.  This will include but not be limited to discussions of beliefs, mystical experiences, personal interactions etc.  The interview will last approximately one hour.

The interview will be audio recorded so that the researcher will be able to review your comments after the interview.  However, if you do not want the interview to be recorded then recording will not occur.  Further, you can request that the interviewer pause or turn off the recording at any time during the interview.  Audio recordings from the interview will not be shared with others or made public, although you can request a copy of the audio recording if you wish.  If you would like a copy of the interview to read, you can also request the researcher to send you a written copy of the interview so that you may edit or delete anything you do not want included as part of the final interview document.

The following steps will be taken to protect participants’ identities during this study: (1) Each participant will be assigned a number; (2) The researchers will record any data collected during the study by number, not by name; (3) Any original recordings or data files will be stored in a secured location accessed only by authorized researchers.  

Participant Requirements  

To complete the online form to the best of your ability. If you choose to participate in a follow-up interview, all questions during the interview are optional and you are welcome to end the interview at any time if you feel uncomfortable.

Risks

The risks and discomfort associated with participation in this study are no greater than those ordinarily encountered in daily life or during or during the course of a conversation. 

Benefits

There may be no personal benefit from your participation in the study but the knowledge received may be of value to humanity. 

Compensation & Costs

There will be no cost to you if you participate in this study.  

Ending the Your Participation

Your participation in this study is entirely voluntary. You are free to refuse to be in the study. 

Confidentiality

By participating in the study, you understand and agree that Dr. Nathanael Homewood may be required to disclose your consent form, data and other personally identifiable information as required by law, regulation, subpoena or court order.  Otherwise, your confidentiality will be maintained in the following manner:

Your data and consent form will be kept separate. Your consent form will be stored in a locked location and will not be disclosed to third parties. By participating, you understand and agree that the data and information gathered during this study may be used by Dr. Nathanael Homewood and published and/or disclosed by Nathanael Homewood to others.  However, your name, address, contact information and other direct personal identifiers in your consent form will not be mentioned by Nathanael Homewood in any such publication or dissemination of the research data and/or results.  

Rights

Your participation is voluntary.  You are free to stop your participation at any point.  Refusal to participate or withdrawal of your consent or discontinued participation in the study will not result in any penalty or loss of benefits or rights to which you might otherwise be entitled.  The Principal Investigator may at his discretion remove you from the study for any of a number of reasons.  In such an event, you will not suffer any penalty or loss of benefits or rights which you might otherwise be entitled.

If you have any questions about this study, you should feel free to ask them.  If you have questions later, desire additional information, or wish to withdraw your participation please contact the Principal Investigator by mail or e-mail in accordance with the contact information listed on the first page of this consent.  

Conflict of Interest

It is important to note that the data provided during this interview will be used as part of published works about Benny Hinn and Global Pentecostalism. 

Voluntary Consent

By participating in this study, you agree that you have read the above information.  You understand that you may ask questions about any aspect of this research study during the course of the study and in the future.  By submitting the form on the web page “Submit Your Story,” you agree to participate in this research study.
